Excellence in Patient Engagement for Patient Safety

HealthCareCAN, the Canadian Patient Safety Institute and Health Standards Organization, with support from Patients for Patient Safety Canada, have partnered to identify, celebrate and disseminate leading practices in patient engagement for patient safety.

Each year, a panel of judges from the three organizations, including patient representatives, carefully consider every nomination to identify the organizations and teams whose leading practices will be publicly celebrated. In addition, two teams, each consisting of a staff member and a patient partner, are selected to present at the National Health Leadership Conference (NHLC). All travel expenses and full registration to NHLC are covered.

Program description

IMPACT

Using quantitative and/or qualitative evidence, demonstrate how the project or initiative has resulted in identifiable or measurable change in patient safety outcomes (in relation to the objectives of the initiative).

ENGAGEMENT

Describe the techniques for engagement, collaboration and partnership with patients/families employed throughout the initiative.

SPREAD & SUSTAINABILITY

Demonstrate how the initiative, its results, and the engagement techniques used are or will be sustained: is there is a sustainability plan, are/ can they be replicated and/or spread to other areas of the organization and beyond?

INNOVATION

Describe how this initiative is innovative in developing or adopting new approaches that improve patient safety through patient- and family-centered care and patient engagement.

PRESENTATION

Describe how this initiative or innovation would make a relevant and engaging topic of presentation at the National Health Leadership Conference.

Terms & Conditions

  • English and French nominations will be accepted.
  • Only one nomination per team will be accepted. There can be multiple nominations from the same organization as long as they describe different initiatives/practices implemented by different teams.
  • Those nominated in previous years can be nominated again if the initiative/practice is different than what was previously submitted.
  • The content submitted must be original.
  • All individuals listed in the nomination must be notified by the person submitting the nomination.
  • All nominations are evaluated by the Award Panel who may request further information. Their decision is final.
  • A note regarding the status of the nomination will be sent to the contact person in early December.
  • All materials become the property of the partners and will not be returned. Application-specific feedback will not be provided.
  • By submitting the nomination, you give the partners the right to make public your team and organization name as well as title and summary of the initiative/practice. Photos and logos may also be requested.
